JOB PURPOSE
The safety investigator is responsible for ensuring that safety investigations are performed as prescribed in the safety investigation procedure, in compliance with the applicable local and international regulations, and to ensure that incident/accident narratives are recorded to identify root cause, observations, findings, and propose effective corrective actions, preventive actions, and recommendations.
KEY ACCOUNTABILITIES
•Reviews the reporting system for all recorded accident/incident occurrences to initiate safety investigations and follow-up correspondence when necessary.
•Carry out investigations into accident/incident occurrences in coordination with the relevant departments and stakeholders, and prepares investigation reports with narrative, root cause analysis, other findings/observations, corrective actions, preventive actions, and recommendations.
•Coordinate the review of investigation reports with management.
•Carry out comprehensive root cause analysis on all occurrences to identify gaps and propose appropriate corrective actions, preventive actions, and recommendations.
•Maintain a database for all accident/incident investigation reports, with list of root causes, types of occurrences, severity of occurrences, and corrective actions/recommendations.
•Prepares monthly and annual analysis reports with number of investigations carried out, types of occurrences investigated, number of investigations by entity, number of corrective actions/recommendations, status of corrective actions/recommendations (open/closed).
•Acts as the point of contact with BCAA and BAC on investigations into accidents/incidents (under the supervision and direction of Manager Aerodrome Safety).
•Provides updates to BCAA on a continuous basis (daily/weekly) on accident/incident occurrences, status of investigation reports, and progress of implementing corrective/preventive actions.
•Submits completed investigation reports to BCAA as and when requested.
•Provides recommendations for improvement to procedures, policies, manuals, guidelines, instructions, and other documents to improve safety.
•Supports with carrying out risk assessments into accident/incident occurrences, procedures, manuals, and/or as part of the change management process with expert input from an investigator’s perspective (i.e. based on the history of investigations, and context of investigations).
•Supports Quality Control with audits on departments by preparing a list of pending corrective actions/recommendations as related to investigations, to be included as part of the audit scope.
•Participates in safety audits as and when required.
•Ensure that the safety promotion campaign incorporates the necessary information as relevant to the root cause and trend analysis of investigations.
•Provide input on triggered SPIs, and SPI development, taking into account safety investigation root cause, trend, and gap analysis.
QUALIFICATIONS
•University degree in aviation management, or equivalent qualification.
•ACI Safety Management Diploma, or equivalent desirable.
•Aviation Legislation and ICAO Annexes training.
EXPERIENCE
Minimum 5 years’ experience with special focus on aerodrome safety and relevant investigations.
